Relisting after graft failure
Intended audience: recipient coordinator transplant program admin
If a transplanted organ fails, the recipient can be added to the waitlist for a second transplant.
Please register a graft failure for the first transplant before adding the recipient to the waitlist for a second transplant.
Registering a graft failure
To register a graft failure:
Go to the recipient’s profile, then select Organ > Post Transplant Follow Up in the left navigation.
Scroll down to the Post Transplant Follow-Up details.
Select the Create Follow-Up button.
Select Graft failure in the Recipient Status dropdown under New Follow-Up.
The system opens fields for input.
Enter information in the fields that appear.
Failure Date.
Failure Cause.
Failure Cause if Other.
Source of information
Source of Information if Other.
Select the Save Follow-Up button.
To cancel, select the return to previous page icon or browse to any other page without saving. Any unsaved information is discarded.
Adding a new transplant journey
Initiate and complete a new journey for the same organ type, as described in the help pages for Referral, Consultation, Medical Assessment, and Waitlist.
If the new journey is for a pancreas or kidney, and if a graft failure was registered (per above) then the system will populate the new journey with dialysis history and donor acceptability criteria from the original (failed) journey.
Reinstating wait time (automatic)
If the new journey is for a pancreas or kidney, and if a graft failure was within 90 days of the preceding transplant, then the system will calculate the new wait time.
When adding the recipient to the pancreas or kidney waitlist, the system provide a prompt:
Click OK.
The added days appear in the Wait time override area of the Waitlist section.
Reinstating wait time (manual)
For organs other than kidney and pancreas, or if the kidney/pancreas failure more than 90 days after the transplant, the transplant program can apply to the Special Case Committee for wait to be added to the recipient’s journey. See Apply Wait Time Overrides.
