Manage exception diseases
Intended audience: recipient coordinator transplant program admin
In addition to MELD or PELD scores, a recipient’s Allocation Score is impacted if they are eligible to receive exception disease points. You can indicate which liver diseases the recipient has so the system can factor them into the exception points calculation.
Liver or liver cluster recipients with exception disease(s) will stop accruing wait time and exception points when placed on hold for a non-medical reason (see Medical Hold and Liver Recipients section). The system suspends the journey if it is on hold for a non-medical reason.
Recipients with an exception disease when put on hold for any medical reason, will continue to accrue wait time and exception points, unless the journey goes into a Medical Suspension.
Adding or updating exception diseases
Go to the recipient’s profile, then select Liver > Liver Specific Details on the sidebar.

Scroll to the Exception Diseases section.

The fields are populated with any previously saved information.

Additional Paediatric Conditions diseases are shown for recipients under 18 years of age.
If any Paediatric Conditions are selected, this information continues to appear even after the recipient’s 18th birthday.
You can unselect these conditions when they no longer apply. If no Paediatric Condition was selected before the recipient turned 18, the Paediatric Conditions section will not be visible once the recipient turns 18.
Select or deselect diseases as appropriate.

Select the Save Diseases button.
To cancel, select the return to previous page icon or browse to any other page without saving. Any unsaved information is discarded.
The system calculates an updated Exception Disease Points score and presents it at the top of the Exception Diseases section.
The system also updates the Allocation Score based on the new Exception Disease score and adds a row to the Allocation Score history table. The new score will be used in subsequent liver allocations.
The system may show an Other Diseases field, if the Special Case Committee approved a disease not on the list of exception diseases. See Apply exception disease override.
