Intended audience: recipient coordinator transplant program admin

Recipient Coordinators can make a request to TGLN to transfer a recipient’s care to their program, from a different transplant program. A transfer may be required if a recipient re-locates within Ontario, or if the recipient needs an additional transplant that cannot be performed at the original transplant centre.

There are several transfer scenarios that are handled differently

  • Active transfer: transfer a waitlisted recipient’s care to a new transplant program.

  • VAD transfer: transfer a heart recipient’s Ventricular Assist Device (VAD) care to a new transplant program.

  • Post-transplant transfer: transfer a recipient’s post-transplant care to a new transplant program.

When a transfer is processed in OATS, some or all of the recipient’s journey information will be made available to the new transplant program.
