In the current version of the system, a VAD transfer is initiated by the destination hospital with a request to TGLN, which is then handled manually by the system support team (Shore).

Requesting a VAD transfer

As a coordinator at the destination hospital, complete the OATS User Support Intake Form to request that a heart recipient’s VAD care be transferred to your program.

Completing a VAD transfer

Intended audience: csc manager tgln admin system support team

At TGLN, raise a service request on Shore’s Service Desk portal specifying details of the transfer.

Shore will complete the transfer.

The Current Hospital field in the VAD Information section of OATS is updated to reflect the transfer.

  • The destination hospital can edit the VAD record.

  • The destination hospital can manage the VAD record going forward.
