The pre-allocation checklist is a quality control step that occurs after an allocation recommendation has been run (or re-run), and before the offer process can start.

Two Clinical Services Coordinators (CSCs) must complete the checklist. This is to confirm that the allocation recommendation accurately reflects the donor information. The objective is to ensure that offers are made to suitable recipients only.

Confirmations cannot be changed once the checklist is saved.


Initiating a pre-allocation checklist

Prerequisite. An allocation recommendation must have been run.

The CSC who creates the allocation recommendation must confirm it is accurate.

  1. Go to the donor profile, then select Organ > Checklists on the sidebar.

  2. Select the Create Pre-Allocation Checklist button.

  3. The system presents a blank checklist. Note the items on the checklist are different, depending on the organ being allocated. (For example, for a kidney being allocated provincially, Ontario IPOS Kidney at Export Threshold? appears.)

  4. For each item on the checklist:

    1. Validate the item by comparing the allocation recommendation to source data (on iTransplant and OATS).

    2. Select the I Confirm checkbox under CSC 1 Confirmation.

  5. Select the Save Pre-Allocation Checklist button.

The system adds the new checklist to the table at the top of the section. The checklist can be verified by another CSC.

Verifying a pre-allocation checklist

Prerequisite: A CSC has completed the checklist and has requested verification. The system will not allow the first CSC to verify any checklist items.

A second CSC must review the allocation recommendation and verify that it is accurate.

  1. Go to the donor profile, then select Organ > Checklists on the sidebar

  2. In the table of previously completed checklists, select the most recent pending pre-allocation checklist.

  3. The system presents the selected checklist, showing the first CSC's confirmations.

  4. For each item on the checklist:

    1. Validate the item by comparing the allocation recommendation to source data (on iTransplant and OATS).

    2. Select the I Confirm checkbox under CSC 2 Confirmation.

  5. Select the Save Pre-Allocation Checklist button.

The system saves the checklist and updates the table at the top of the section.

The allocation’s status changes from Allocation Run to Offering. A CSC can begin making offers of the consented organ.

Viewing checklist history

To see the status of checklists for a given allocation:

  1. Go to the donor profile, then select Organ > Checklists on the sidebar.

    Example - Check list controls and table of completed checklists.


  2. Select Checklist History at the top of the table.

  3. The system presents a pop-up window.

  4. To close the window, select Close, select the small red x, or press Escape.

Viewing a pre-allocation checklist

Checklists can be viewed by TGLN management and auditors to confirm that the allocation and offer process was conducted properly. CSCs and CSC managers cannot open and view completed checklists.

  1. Go to the donor profile, then select Organ > Checklists on the sidebar.

  2. In the table of previously completed checklists, select any pre-allocation checklist from the table.

  3. The system presents the selected checklist, showing CSC confirmations that have been completed.
